"How To Infiltrate? A Survival Guide"
Infiltration is a technique that was proposed when a new product (Icon-DMG), designed to halt caries in the posterior teeth, was launched on the market. At the first stage, we perform superficial demineralization by application of a 15% solution of hydrochloric acid, a technique that opens up access to the hypomineralized lesion; then, in a second stage, an extremely fluid resin is infiltrated into the body of the lesion. A consequence of infiltration technique is that it masks the white spots of enamel caused by different etiology in anterior teeth.
In this presentation we will focus in the differential diagnosis of white spots of enamel and the different characteristics of these lesions. We will explain in detail the basic protocols of infiltration and deep infiltration techniques and learn how to identify difficult MIH lesions and their topography.
Finally we will understand why we fail, what are the challenges in infiltration technique and get ting clinical tips about predictable results.

"Direct restorative therapy from 5-95"
There is not too much of a difference whether you treat a 5-year old or a 95-year old patient. This lecture covers fundamental aspects for restorative dentistry over the whole lifespan from treating deciduous teeth to the very far end called root caries. First of all, it is important to master different adhesive techniques in different biological substrates, facing surprising differences between the first and the second dentition. Direct posterior restorations including deep margin elevation cases as well as repair will follow. Finally we have a deep look into realistic esthetics of anterior teeth, from trauma to erosion and finally root caries. Our concept is called “Composite first – Ceramic second”, not because composite is the always superior material, but relating to the age of the patient. The young patient will get 95% direct restorations; the older may receive more and more indirect restorations. Finally a few tipps and tricks for root caries lesions are presented.

"Myofunctional Therapy in Pediatric Patients: A Clear Approach to Craniofacial Development for a Healthy and Functional Smile"
Myofunctional therapy, combined with the use of silicon preformed appliances, offers a unique and effective approach to the treatment of orofacial myofunctional disorders (OMDs) in pediatric patients. This innovative approach integrates principles of epigenetics and craniofacial development to promote optimal growth and development, while addressing the underlying causes of OMDs.
During the pediatric growth period, the craniofacial complex is highly plastic and susceptible to the influence of environmental factors, including muscle function. Incorrect orofacial muscle habits, such as mouth breathing and atypical swallowing, can exert abnormal forces on the developing dentition and jaws, leading to malocclusion and other OMDs.
The silicon preformed appliances are designed to work synergistically with myofunctional therapy exercises to:
1. Correct muscle habits: The appliances apply gentle, targeted pressure to the orofacial muscles, promoting their re-education and retraining in correct function. This helps to normalize muscle tone and establish a balanced orofacial environment.
2. Facilitate craniofacial development: By guiding the muscles into proper function, the appliances create optimal conditions for the harmonious growth and development of the jaws and dentition. This allows for the natural eruption of teeth into correct alignment and promotes the development of a healthy and functional craniofacial-complex.
3. Modulate epigenetic patterns: The mechanical forces generated by the appliances and myofunctional exercises can induce epigenetic changes, influencing gene expression and cellular processes that are critical for craniofacial development. This epigenetic modulation enhances the potential for favorable treatment outcomes and long-term stability.
The combined use of silicon preformed appliances with myofunctional therapy in pediatric patients offers a range of significant benefits, including:
Improved breathing and sleep: Correction of mouth breathing and establishment of nasal breathing promote oxygenation, improve sleep quality, and reduce the risk of sleep-disorderedbreathing.
Enhanced craniofacial development: The appliances guide the growth and development of the jaws and dentition, promoting optimal facial esthetics and function.
Reduced need for orthodontic treatment: By addressing the underlying causes of malocclusion, the early intervention with silicon preformed devices can potentially reduce the need for future orthodontic treatment.
Improved quality of life: The resolution of OMDs and the development of a healthy smile can significantly improve a child's self-esteem, confidence, and overall quality of life.
Myofunctional therapy with silicon preformed appliances represents a groundbreaking approach to the treatment of OMDs in pediatric patients. By integrating principles of epigenetics and craniofacial development, this innovative method offers a unique opportunity to optimize growth and development, while achieving a healthy and functional smile. The benefits of this approach extend beyond the correction of OMDs, contributing to improved overall health and well-being in children.

"MIH – A worldwide challenge"
Molar incisor hypomineralization (MIH) is a frequently encountered dental condition worldwide. It is defined as hypomineralization of systemic origin of one to four permanent first molars frequently associated with affected incisors. Affected teeth are more prone to caries and post-eruptive enamel breakdown and should be diagnosed and managed as early as possible. Furthermore, tooth hypersensitivity is a common symptom in these patients. The etiology of MIH remains unclear. Regarding diagnostics and classification of MIH affected teeth, the EAPD criteria can be used. Furthermore, the Wuerzburg concept offers a classification index – the MIH Treatment Need Index (MIH-TNI) – and a treatment concept based on it. Available treatment modalities for MIH-affected teeth range from prophylaxis to restorative approaches and possibly even extraction. The choice of treatment option depends on a number of factors. These include the severity of MIH, the presence of symptoms, the age of the patient, and the social background and expectations of the child and the parents.
This lecture aims to highlight different aspects related to MIH, from its etiology, diagnosis and classification to current treatment options.

"A New Method for Dental Biofilm Management in Children: Guided Biofilm Therapy (GBT)"
The main cause of dental caries and periodontal diseases is the biofilm layer that forms on the surfaces of natural teeth and contains dense pathogenic microorganisms. With the understanding of biofilm-related destruction mechanisms of tooth structure, it has been reported that the primary approach in the treatment of dental caries and periodontal diseases should be biofilm eradication treatments.
Nowadays, different new techniques and methods have been introduced for the mechanical eradication of biofilm layers and hard attachments on early caries-affected tooth surfaces and for the effective control of biofilm formation. In this regard, a new sequential treatment protocol called guided biofilm therapy (GBT) has been introduced and highly successful clinical results have been found. The basic philosophy of GBT is to achieve more effective and non-invasive biofilm eradication together with the clinician and the patient, as well as long-term motivation of patients and optimal home care processes.
One of the most important and fundamental steps of GBT is the use of biofilm staining sponges, and it is very useful for both the patient and the clinician to make the areas where the biofilm has accumulated visible.Other important steps of GBT are systematic professional prophylaxis procedures using air, water, powder abrasives and ultrasonic technologies in biofilm management.GBT also enables individualized treatment protocols with different diagnoses and risk assessments for each patient to achieve optimal clinical outcomes.
This presentation will review and discuss the methodological algorithm changes in biofilm management to achieve effective results in the prevention of dental caries and periodontal disease.

"Soft Tissue Laser Applications"
Laser is enables optimal preventive, interceptive, and minimally invasive treatment interventions for both hard and soft tissue procedures in pediatric dentistry. Studies showed that soft tissue laser, such as diode and Nd:YAG lasers were increased success rate of treatment in soft tissue lesions. In this presentation, case reports of the usage of soft tissue laser in pediatric dentistry will be explanied.
Case 1: Lingual frenectomy, Case2: Labial frenectomy, Case3: Gingivectomy , Case4: Primary tooth amputation, Case5: Permanent tooth amputation, Case6: Deep margin elevation after trauma, Case7: Exposure of unerupted teeth.
The benefits of laser usage are the less thermal necrosis, hemostasis can be obtained without the need for sutures, wound healing to occur more rapidly, less post-operative pain, demonstrate decontaminating and bacteriocidal properties on tissues and requiring less prescribing of antibiotics post-operatively. Although soft tissue laser is defined as an acceptable, easy, less time-consuming and comfortable treatment method, there is a need for larger studies and case reports in the use of soft tissue laser in pediatric dentistry.

"No Post no Core approach in the Restoration of Endodontically Treated Posterior Teeth;
Endocrown Restorations "
The long-term clinical success of endodontically treated teeth is closely related to the endodontic treatment as well as successful coronal restoration that does not cause re-infection of the root canal system. While successful coronal restoration prevents fracture and coronal leakage of endodontically treated teeth, it also ensures that the dental hard tissues are supported and the tooth functions against chewing forces. However, since the biomechanical changes that occur as a result of endodontic treatment negatively affect the long-term prognosis of these teeth, coronal restoration of endodontically treated teeth, especially those with high structure loss, is still very challenging for clinicians in clinical practice. The most commonly used restoration type in the clinic for these teeth is still post and core systems performed together with crown restorations. However, while post systems increase the retention of the coronal restoration, they also increase the risk of root fracture. Evidence-based dentistry data shows that, together with the development of adhesive materials and the minimally invasive dentistry approach, endocrown restorations, also called as "no post no core approach", are a reliable alternative to post core crown restorations for posterior teeth. In this presentation based on clinical trials and evidence-based dentistry results and using clinical cases, indications, preparation designs, luting procedures, and success of different types of endocrown restorations fabricated from various restorative materials will be discussed.
